Change of Name of the Insured on the Policy Document

 Circumstances under which Name Change on LIC Policy is allowed

1. If a woman wants to change her surname post marriage
2. If a couple has adopted a child and wants their surname after his/her name (legal adoption)
3. If the insured has his/her name legally changed for a specific reason
4. If the insured has his/her surname legally changed for a specific reason
5. If the name of the policyholder has some disparity (misspelling)

Important Points to Keep in Mind:

1. Name change can only be done by LIC officials from the home branch
2. Documents such as passport and PAN card are typically required during a name change
3. Consulting a life insurance expert before commencing the name change process is advisable to avoid errors
4. Original copies of documents along with affidavit must be carried along at all times

Visit the nearest Branch and kindly carry the following documents:  

1.    Request Letter   

2.    Standard ID proof with DOB & correct name.
3.    Marriage certificate (If name change due to marriage)
4.    Gazette notification (In case of complete name change)
5.    Standard ID proof of Proposer.
6.    Original Policy
