Change of bank account details & Nach Procedure

 

Requirements:-

  1. Request application from life assured or proposer ( if life assured minor )
  2. Properly filled NACH form 
  3. Cancelled cheque with printed name on it OR Verified Bank passbook xerox copy .
  4. KYC ( Pan Card , Adharcard )
  5. Latest premium should be paid before submitting request
